Coulter’s Hands provides compassionate monitoring and support for changes in comfort, pain, breathing, restlessness, appetite, nausea, agitation, weakness, sleep patterns, and overall decline. Our experienced care team helps families recognize changes early and communicates concerns promptly to the primary hospice provider or physician as appropriate.

Coulter’s Hands develops individualized care plans based on the patient’s needs, goals, preferences, routines, safety concerns, and family priorities. Care plans are created in collaboration with the patient, family, and primary care or hospice team when applicable.

Coulter’s Hands provides education and support to patients and families regarding disease progression, comfort-focused care, symptom changes, caregiving techniques, safety, and what to expect throughout the care journey. Our goal is to help families feel more confident, prepared, and supported when caring for their loved one.

When ordered and appropriate, Coulter’s Hands nurses may assist with medication administration and monitoring for effectiveness, comfort, and safety. Our team observes for uncontrolled pain, anxiety, shortness of breath, restlessness, or other changes and communicates concerns to the primary hospice or medical provider. Comfort medications remain ordered and managed by the patient’s primary hospice or healthcare team.

Coulter’s Hands provides emotional support to patients and families through compassionate presence, active listening, encouragement, prayer (when welcomed), and a calm, reassuring approach during difficult seasons of care.

Coulter’s Hands may help coordinate or offer comfort-focused supportive services such as music, gentle hand massage, aromatherapy, pet visits, hair appointments, or other meaningful activities that promote comfort, peace, dignity, and emotional well-being. Availability may vary and some services may be offered as add-ons.

Coulter’s Hands offers access to experienced nursing guidance and support to help bridge the gap between routine provider visits. Our team is available to answer questions, provide direction, and help families know when additional support from the primary hospice or medical provider may be needed.
