Hospice and Palliative Support

What is hospice?

Hospice is a type of care focused on providing comfort and support to individuals with terminal illnesses, typically when curative treatment is no longer an option or desired. The primary goal of hospice care is to enhance the quality of life for patients and their families by managing pain and symptoms, addressing emotional and spiritual needs, and offering support through the end-of-life journey.

Our goal at Coulter’s hands is to help people live as fully and comfortably as possible for the time they have left, surrounded by the people they love most. We offer an in-home service unlike any other by adopting the philosophy of co-created care, we empower our clients and families to actively participate in their care journey.

We believe this model extends beyond nursing and is about building relationships, encouraging open communication, and providing education without over-complicating things. This philosophy guarantees that every care team member has a voice that is not just heard but truly valued.

What is end of life?

End-of-life refers to the final phase in a person’s life when they’re approaching death. This period is characterized by a range of physical, emotional, and spiritual changes and is often associated with terminal illness or advanced irreversible conditions.

End-of-life care focuses on supporting the individual and their loved ones while providing comfort in all of life’s final moments.
